Radiologic Imaging in the Management of Sinusitis -- American Family Physician

چکیده
www.aafp.org/afp VOLUME 66, NUMBER 10 / NOVEMBER 15, 2002 plete opacification are more specific for sinusitis but are seen in only 60 percent of sinusitis cases. Interpretation of plain radiographs can vary widely among different observers, and there is a high rate of false-negative results. Radiographs of the sinuses in infants aged three years or younger are not useful because of false “opacification” from undeveloped sinuses. Other important limitations of plain radiographs include poor visualization of ethmoid air spaces and difficulty differentiating between infection, tumor, and polyp in an opacified sinus. Because clinical judgment is sufficient to diagnose sinusitis in a majority of cases, and empiric treatments are inexpensive and safe, only a small percentage of patients who develop recurrent or complicated sinusitis are candidates for imaging studies. Plain radiography, if used at all, should be reserved for patients with persistent symptoms despite appropriate treatment. A single Waters’ view (occipitomental) appears to provide as much information as the standard four-view series.

Radiologic evaluation of acute chest pain--suspected myocardial ischemia.
The American College of Radiology has developed appropriateness criteria for a number of clinical conditions and procedures. Criteria are available on imaging tests used in the evaluation of acute chest pain--suspected myocardial ischemia. Imaging tests for a suspected cardiac etiology include transthoracic echocardiography, transesophageal echocardiography, radionuclide perfusion imaging, radi...

The increasing use of cross-sectional imaging has led to an increase in the incidental discovery of adrenal masses (adrenal incidentalomas). Although most of these lesions are benign, they often present a diagnostic dilemma. Before creating a management plan, the physician should determine if the lesion is benign or malignant and if the lesion is functioning or nonfunctioning. Incidentally disc...
متن کاملRadiologic evaluation of hematuria: guidelines from the American College of Radiology's appropriateness criteria.
Hematuria, symptomatic and incidental, that involves more than three red blood cells per high-power field on two of three properly collected urinalysis specimens warrants some type of imaging to evaluate the upper tracts. Traditionally, excretory urography or the intravenous pyelogram has been the mainstay of the hematuria work-up, but computed tomography urography has more recently been recogn...
متن کاملHow family physicians distinguish acute sinusitis from upper respiratory tract infections: a retrospective analysis.
BACKGROUND The purpose of our study was to examine how physicians diagnose sinusitis in practice. We addressed three specific questions: (1) what clinical factors do physicians look for in evaluating and caring for patients with suspected sinusitis, (2) to what extent do physicians use transillumination and radiograph evaluations in diagnosing sinusitis, and (3) how does the diagnosis of sinusi...
متن کاملRadiologic Imaging in Chronic Sinusitis
Sinusitis is one of the most common diseases which primary care physicians encounter in their daily practice [1]. Nowadays, it’s widely approved that radiologic imaging is not indicated to evaluate uncomplicated sinusitis cases. Further diagnostic evaluation and radiologic imaging should be reserved for sinusitis cases whose symptoms persist or recurr despite appropriate treatment. Sinusitis ca...
